BACKGROUND AND PURPOSE: The below publication presents a case of a 51-year-old patient with cervical discopathy of unusual clinical course. CASE REPORT: The symptoms of the disease suddenly became aggravated and took a form of meningeal syndrome without inflammation of cerebrospinal fluid. The authors emphasize the symptomatology and diagnostic difficulties connected to unusual clinical course of cervical discopathy at the level of VC3/VC4. DISCUSSION: Both medical and neurosurgical approaches to clinical history of cervical discopathic patient were presented in details. The patient underwent anterior cervical interbody fusion and the operative procedure was very effective for a few years up till now.

A patient with surgical and neuropathologically confirmed subdural sarcoid granuloma was evaluated using angiography, contrast-enhanced CT and MR. MR images were obtained on a superconducting magnet with T1, intermediate and multi-echo T2 weighted sequences. Review of the 2 prior cases of subdural sarcoid granuloma from the literature evaluated with MR confirm the variable nature of signal intensities of the lesion. MR was most useful in anatomically evaluating the lesion and planning neurosurgical intervention but both CT and MR alone, in this case, did not definitely obviate the other differential diagnosis including meningioma en-plaque, lymphoma, or metastasis.

A case of spontaneous pneumocephalus with progression to tension pneumocephalus is presented. It is unique because it occurred spontaneously via a sellar defect 5 years after resection and irradiation of a pituitary adenoma, without tumor recurrence. Computed tomography clarified the extent of the pneumocephalus and its progression to tension pneumocephalus.
